7. Application Procedures for Attending, Presenting
(1) Presentation applications: fill in the necessary information on the application form

     and send it to the conference Central Office (Suzuki)
     *Applications for video presentation of "My Class" are due by Friday, April 27th.
     *Applications for presentations, panel discussions and symposiums due by Monday, May 14th.
     *Write the presentation topic in both Japanese and English
     *In addition to the above application form, please assist us by sending the presentation topic,the name of the presenter(s) and their institutional affiliations by e-mail. Do not send it in an attachment;  send it as a simple text file. E-mail applications will not be accepted.
(2) Applications for attending the conference should be sent to the conference Central Office (Suzuki) by Friday, May 25th.

8. Hotel reservations, Lunch and Reception:
@@Hotel and lunch reservations are the individual's responsibility, due on Friday, May 25th.   Fill out the necessary information on the reservation form for hotel, lunch, and reception(on a separate sheet) and send it eitherby regular mail or by fax to the address below:
